Fluval 3+ Always Clogs Up

Any ideas why? It's running on a 13 Gallon tank for now until I set up my 20 Gallon within the next few days, I have plants that aren't in the substrate, just hovering, but I don't think that's the problem. If anymore information is required, just ask.
