Kohaku swordfish.. what is this?

I just came into some kohanu swordfish. I'm wondering what this is? Poop? Gravid spot? Worms? In the light it looks almost maybe an orange color? Definitely internal though. We have 2 females that came from a mixed tank.

check the water quality for ammonia, nitrite, nitrate and pH.

do a big (75%) water change and gravel clean the substrate.
make sure new water is free of chlorine/ chloramine before it's added to the tank.
