Did you yourself test tap water? When I did my nitrate was 20ppm. So my tank always has 20ppm and up to 30 or 40. I keep an eye on it and do water changes as it gets high.
thank you for your interest, the tap water has been tested and is fine, all i can think is something has contaminated this tank (obviously) and I have taken every precaution. just going to empty and start again and be uber careful. weird as hell!

Hopefully a fresh start helps. I do think it's useful to purchase your test kit so you can monitor water levels. Sorry I can't be of more help.

and yes, kit has been ordered, lessons learned...
 
If the reverse osmosis water is good (the cartridges in them need changing from time to time) and the tap water is good, then the nitrates are probably coming from the substrate.
What is the substrate?

You can test the substrate by putting it in a bucket of tap water. Check the water quality before adding it to the bucket, and again 24 & 48 hours later, and a week later. If the substrate is releasing something, then the water will change during that time.
