Silly question, but would it be possible to restrain the fish and scrape the cysts off? Since he only has two? And then treat for possible bacteria infection (as you'd be leaving small wounds behind)? Or would that just do more harm than good?

I don't have any direct experience, but I don't think that the above is a good idea. I'd just set up a tank with those two fish and let them live out the rest of their natural lives.

I figured not...it's just I can only have these two tanks, and this means I won't have a extra tank and will have to take the risk of adding new fish to my main tank without quarantining them.

But if I have to keep this guy separated, then I'll just have to live with it (and so will he). I think I'll run him through a course of treatment for Columnaris since he does also have the very mild signs of fin rot. From what I'm reading on Columaris though, he should be much worse after having had it for so long.

I'm still hoping not...the spots don't look quite like any of the pictures, and he has no other symptoms. He's also been stable in this condition for many months, which I don't think would be typical for the virus. I know if it is indeed dwarf gourami disease there's nothing I can do, but as long as he's eating and active I'm going to hold out some hope.
 
There is a rare fish virus called Lymphocystis which causes white spots but it mostly affects the fins of the fish (though can be on the body too);
