My ADF Died a nasty death!

OK I was planting my tank a few days ago, and everything seemed fine. I went to rinse out my filter media ( I use an internal fluval 3 plus I'm sure a few people use these) and i found one of my ADF's trapped in the filter. I've got no idea how he got in there.

There are two sections to this filter which come appart, the first part is the impeller and outflow and the other is the media box. Anyway I opened it and found an ADF just under the impeller, it wasn't chopped up, just very squashed
