I'll blame the parasites. They may we have done serious damage to the gills of the fish, and that would have left them vulnerable. When you have a creature that drills into a fish to extract nutrients, the issues don't go away when the parasite dies. It takes time for the damage to heal.
Camallanus nematodes kill fish after successful treatments, as they do really serious internal harm to their hosts. Ich does its share of damage too.
