i set up a tank at the start of the month for my shrimps, at the same time i added 3 otocinclus and a couple of corys.
after about a week 1 oto died
from unknown reasons, just 1 of those things with otos i guess! then i could only find 1 of the 2 remaining. for some reason ive just checked in the in hood filter in the aquaone ar620 tank and there he was stuck in the floss 
i thought he was dead but when i touched him he wriggled he must have been in there for at least a week and had burrowed into the floss getting all tangled up. to releas him i had to gently pull on his tail fin as he was also very determined to stay there.... after getting him out and removing the remaining floss from him he went very pale and lifeless when back in the tank and again i thought he had died. but he started moving and his colour has come back a little.
anyway my question is how the hell did he get there?
the intake for the inhood filter has a sponge covering the strainer as the tank is really for shrimps so preventing them getting sucked up. even if he did get sucked up which is pretty impossible he wouldnt have got through the spraybar inside the hood. the other end of the filter is obviously the out flow of th system so he couldnt have made it in there either.... there are 2 small holes 1 at each end that he could have swam into but then he would have to climb out of water about 3 inches maybe more to get into the top of the filter into the floss
do you think he will be ok?
