Sick betta not eating and laying at bottom

My betta fish that I've had nearly 3 years hasn't eaten the past 3 days, and is just laying at the bottom of the tank, looking almost dead. I did a 25% water change yesterday, but the only time he really seems to swim up is to get air. And recently it has been in very fast corkscrew like out of control turns. He still looks basically the same though, but things aren't looking good. I've always thought he is partially blind, since he's always struggled getting his pellets at the top. It's only gotten progressively worse, with his aim way off the past week. Any ideas on what it is or how to treat him? It doesn't look like he's going to make it. Could it just be old age?
 
What are your water parameters? Does he look like he can't stay afloat / sinks to the bottom?
