Hi and welcome to the forum :)

How long has the fish been like that?
How quickly did it get fat like that?
Is the fish eating well?
What does the fish's poop look like?

If the fish got fat over a day or two, it could have dropsy. However, this normally causes the fish to stop eating.

If the fish got fat over a month or more, then it probably has a tumour and there is no cure.

Hi Colin, thanks for getting back to me.Its happened over the last 3-4 days, his underside is almost transparent its so swollen. He is very active and eats,as of yet haven't managed to see any poop.
I actually drained the fluid build up in the sack on his underside but this morning it's back.
Any more ideas I can try?
Thanks Adam

No idea what else you can do. The fluid and speed with which it appeared would suggest bacterial rather than a cyst. However, bacterial would normally stop the fish eating. If it's a cyst, there isn't much you can do besides draining it but if it comes back, well that's not going to help.

Normally I would say euthanise the fish but if it's still eating, you can keep it alive and see what happens. But it will probably end badly.
