I'm fairly certain it's not ick, not positive though, so far it's the only fish affected but he has white dots just around his eyes and like in the middle of his forehead area they look taller and more like a wart than ick typically does.  Sorry my pictures aren't fantastic but unlike his usual self, he's being hard to photograph

I used to have two gold rams.
One developed a white dot on his side which grew in size and turned out to be a fungal infection. Unfortunately the treatment we tried didn't work and we lost him :( 
 
I am not as experienced as many on this forum so I'm sure others will be able to offer more advice and treatment options but I would definitely look into the possibility of it being a fungal infection. Like i said, it started as a white dot, which then grew and started to look fluffy. 
 
Fortunately no other fish in the tank were affected in my case. (I think part of the problem from the start was the fact that the water parameters in the tank weren't quite right for the ram's (as they need very particular water conditions which my partner didn't realise when he got them) This over time lowered their immune system and made them susceptible to disease.
 
Anyway I really hope you get to the bottom of it and he gets better :) 
 
I hope I figure it out too! other suggestion I got was that it's hole in the head, which, is also bad!
 
It is the only ram in the tank? or is it part of a pair? HITH can appear purely down to stress, if the fish is being bullied he can get HITH, remove the fish to a quarantine tank and do small daily water changes, the fish should start to improve if it is not stressed. Curing mild HITH can take weeks with just water changes, but it can be done. If it is down to stress you need to watch and see who is bullying/stressing the ram. IME with rams they are stressy little fish at the best of times :(
