Self explanatory really.

There are no signs to say it is sick well visible anyway. It is only half the size of the rest. When feeding time comes it seems to eat but then spits it out again.

Is it just ment to be this size or is there somthing wrong.
 
Is the fish a neon tetra, if so what does the red stripe area look like.
Does the red stripe area look bleached out.
Size of tank in gallons or litres.
How many fish and which type.
Water stats in ammonia, nitrite, nitrate, and ph.

Spitting food out can be don't like the brand of food, gill flukes, internal parasites.
What do the gills look like.
Being skinny can be old age, fish tb, not feeding enough, fish stopped eating, internal parasites.
 
Hate to bear bad news, but mine all went like this within 2 weeks of each other. They were all bought at the same time, and were simply very old. Most Pensioners get skinny.. they lose interest in the boring usual food - try feeding treats like small daphnia and hatched brine shrimp. It may take them.

Yeah - look to see if fishies colour has gone as above. Is its back bent?
