sick Beta... please help

Bash has had a hard time lately, been moved a couple times. ammonia spikes.. . and I've noticed his color fading the last couple weeks. He's rubbing himself on plants. . I got a close up pic... somethings wrong... help!! his normal color is black /gold under tones

He’s a beautiful mustard gas. Sometimes color change is normal. Do you see anything on him that he could be scratching?

yes... he seems to like to swim thru the plants... i did treat him with aquarium salt for several days..... but his color is worrying me. he used to be very black... and he just looks unhealthy. The water perameters are good... but he's been pretty stressed lately.
 
What size tank and how often do you do water changes and how much? What temp is his water? What are his water parameters?
 
I just moved him to a larger cycled 5 gallon tank.... his first tank was terrible. but he loved it. It kept getting ammonia spikes.. so i cycled a larger tank.

ammonia - 0
ppm -5
nitrites-0
i usually do weekly water changes. i know he's been very stressed. still eating and active.
but color is dramatically muted..

I take it PPM is nitrates?
