Rainbow trouble.

My Rainbowfish is all bloated and has been for about 3 days, one of his gills is all swollen and popping out and he is breating heavily (not hovering at the surface though). Today I noticed a long white string hanging out of him (does not look like poo)....could it be some kind of worm HELP!!!
