Well when I move in January into my new house, we are setting up a 90 gallon aquarium which will be seen from both sides.

I have 3 Questions:

1. Since the tank will be seen from both sides What filter should I use. It will not have a stand so I will not be able to use those "under the stand filters" as I like to call them, and dont really want to put an external filter on it as it will ruin the look. You guys have any answers?

2. In this aquarium I will also be having sand, and I was wondering if I could have the filter pads underneath sand?

3. What is this?

Undergravel filter is out with sand. There are internal filters however you can use many can be stuck to the glass in the back or on the sides. I have a spare one running in my 10g with my Kuhlis that works fine, besides the occasional suction-cup slip up. Other types can sit on the bottom in the corner and do the job as well.

That is a powerhead. Usually it's for providing a current, waterfall when raised slightly above water or filtration when connected to an underground filter.

Is the tank going to be sitting on the floor, if so, why? DIY tank stands are very cheap and easy.

You may want to look at some sort of canister filter, also if you must use a HOB filter of some sort you could try placing it on the end of the tank, possibly towards a wall.

And your picture appears to be a powerhead.

It really depends on the filter. Some are very unobtrusive and out of the way while others are ugly but functional. They even make them with "camouflage" so they look like ornaments or plants. :)
