More Problems With My Gourami!

As you can see from the pics my Dwarf Gourami has a sore mouth. He had this once before a few weeks ago but it seemed to clear up on it's own. It looks worse this time around so I would like to give him some treatment for it. Can anyone tell me what is wrong with him and what I can use medication wise? He likes to pick at the gravel so this probably doesn't help! He is acting normally and is eating well etc. Other info:

Tankmates are in my signature

Stats: Ammonia 0, Nitrite, 0, Nitrate 20

Tank temperature is 24 degrees celsius

I do a 20% water change once a week and the only thing I add to the water is Tetra Aquasafe Water Conditioner

Is the gravel sharp.
Can you issolate him and try a bacterial med.
Does it look like a growth.

I wouldn't say the gravel is sharp, the pic with the Gourami in the coconut cave will give you an idea of what it is like.

I have my old 48 litre tank up in the loft that I can get if need be. To me it doesn't really look like a growth, more like part of his lip is coming off! I have been reading about Columnaris, do you think it could be this? What bacterial med would you recommend?

Need to add the med fast if the mouth is beginning to rot away.

Could he of damaged his mouth on something in the tank.

I am off out in a sec to get treatment. If the disease can spread then am I best treating the whole tank or should I put him in the spare one I have? There is nothing in the tank that I can think would have damaged his mouth, he had a bump on his lip a while ago but it went away on it's own so I thought no more of it. It does look quite sore so him picking at the gravel probably won't be helping!
 
It's best to issolate sick fish.
As fish can build resistance up to meds so they no longer work.
 
This morning I noticed that my gourami now has a red sore on his side, this is the only picture I could get! Is it realted to his sore mouth?

You need to get going with a bacterial med.
 
Nope he doesn't do any of that, still acting normal and eating well etc. Stats are still the same. Would Melafix be any good, or Pimafix? There are so many out there it is hard to decide which to use!!

Watch out for white edging to the sore as that can be an ulcer.
 
Right I got some Interpet Anti Internal Bacteria Med (no. 9) and have added it to the tank. Fingers crossed he gets better! Thanks for the help Wilder! :good:
 
Right, I added the 2nd dose of Interpet No.9 on Saturday, the Gourami didn't look like anything had changed (no worse but no better either). I came home from work today and his lip has gone all red again and the sore on his side looks worse as well! I didn't turn the lights on this morning so I couldn't say whether it was like it then. Water stats are all the same, here are some pics:

Is there anything else I can do?
 
The only option left is antibiotics from the vet.
You would need to use them in isolation as the antibiotics wipe beneifical bacteria out in the filter.
