An anaerobic pocket would not cause these deaths. The gas that forms there denatures the moment it touches oxygen.
If there was a nitrite spike caused by a lack of NOB then the nitrates would not be being produced during the spike, hence they would be lower.
The problem with this theory is that there are no nitrites now, nor are there any elevated nitrites.
It could possibly be a reaction from the treatment, I guess.
