Help With Fish Fin/scales

HI, recently i have noticed my betta has a largish area on its back where the scales have lost their blue colour and some of the fin strands are quite shabby, This is my first betta and have only had it for about 4 month, if anyone has any advise or can help i would much appreciate it. Can you possibly get a pic from above as well?
 
Also what size is the tank? Temperature? Water parameters? (pH, ammonia, nitrite, nitrate)
Tankmates, if any? Water change schedule? How much/how often.
