First Water Parameters Before Fishless Cycle - Have I Done This Correc

So I am using the API freshwater master test kit and followed the instructions using the water in the tank which is my tap water plus dechlorinator - is that the correct sample, or should just tap water be used?

My readings are:

pH = 7.6
ammonia = 0ppm
nitrite = 0ppm
nitrate - 5ppm

Are these readings suitable??

How do you work out how much ammonia to put in the tank to begin the fishless cycle?

Those water sample parameters are just fine. Tap water plus dechlorinator should be used upon initial setup, and all future water changes thereafter. I believe you add 3ppm of ammonia to start with, but take a look at the Beginners Resource Center > Fishless Cycling.
