and looking at your photos- I have exactly the same problem. I too have no idea what the cause is, but the fish all seem fine and although my Nitrate readings are a little higher than yours, they're still within safe levels.
I carry out 20% water changes every week and vac the sand substrate at the same time. Every couple of weeks I clean under rocks and wood etc, to get rid of anything there. So I think, like you, I do my best to keep things hunky-dory, but then this white cloudy stuff seems to appear from nowhere. Your pictures certainly look like the same thing- close up, lots of tiny particles,almost like minute bubbles.
I only have my lights on from 6pm till midnight, so I should have thought that would rule out over lighting as a cause of algal bloom. And my spraybar is quite high, so I'm getting plenty of surface agitation.
I've tried Bio-Clear, which seems to have made no difference whatsoever. When I do my water change on Sunday, I'll try some Filter Aid and see if that works.
I'll keep you posted- and if you work out what it is and how to get rid of it, please let me know!!

in the end I went for the 'easy approach' and left the tank for around a week and 6 days without touching anything at all and it started to clear by itself
