i need to stop buying plants....

my blyxa japonica is dieing, tank specs, are:

size: 60L
filter: fluval 105
tank at 24[sup]o[/sup]c
substrate: play sand
lighting: 2x 24W t5
CO2: DIY yeast system. 4 2L bottles (changing one a week)
ferts: EL dosing (dry salts)

fish:
blue pearls,
assassin snails
otocinculs
PEST SNAILS (pond/tadpole snails)


here's some pic's what do you think is killing it?/ eating it... and what should i do about it, its not CO2 as my HC is thriving, i have had a little melt on some of my other plants though, but they were new to the tank

Mine done this when I changed substrate but soon bounced back. The roots look fine mate so finger crossed.
 
thanks the thing is nothings changed since set up and it was doing fine before
 
tell me about it, i hope it recovers, some other plants dont have roots where they used to though,
