Betta Can't Stay Upright

I have had this fish for about a week or so now and we've changed his water twice and we feed him a small amount of food twice a day. We noticed early this morning his tail seems to be all twisted and there are a few holes in his tail. Now, he is obviously still alive, but he can hardly swim, won't go below the top inch of the water, hasn't eaten at all today, and he can't stay upright for very long. Anybody know what to do that will help him?

We'll need information to figure out what exactly is going on with your betta.
 
What is the tank size? Do you have a heater? What is the temperature of the water?
Do you have a test kit? Can you give us the readings for pH, ammonia, nitrite, nitrate?
 
Did you cycle the tank? If you don't know what cycling is you can read about it HERE.
 
Can you get a picture of your betta? You can upload one by going to 'More Reply Options' at the bottom of the page and attaching files.
If this doesn't work for you you may want to upload the picture to a photo sharing website such as photobucket and then link it here.
 
What do you usually feed? How much and how often?
 
Sorry for all the questions but they will help give us a clearer picture of what is going on :)
