another cloudy water problem

had my tank for two months now, never had much problem with until now hope someone can help.

About two weeks ago i treated my tank for high nitrares like the pet shop said with pellets they gave me and a 20% water change.

Ever since then the water has been really cloudy. ( whiteish ) I've since done another change this time about 25% which helped a little but after five days it's back to cloudy, also my fish are dieing, the only fish left are tetras ( assorted ) and some small sucker fish. Nothing i do seems to work it's still cloudy and the fish stay at the top most of the time. Hope someone out there can help.
 
Sounds like you have a bacterial bloom but to be on the safe side we are going to need you to get some tank readings so we know where to start. This way we can lead you in the right direction

Water was changed last night and today the cloudiness is better also done water tests,
heres the results........
NITRATE 0.3
AMMONIA 4.9
PH 7.6
Temperature is a steady 28 degrees C
