Can anyone help me identify what's causing this?

He is one of ten white clouds I've had for about 4 years, and has suddenly become tiny, deformed and inactive, however he is still eating. Checked and re-checked my water quality, no ammonia, no nitrite, correct ph... I have had a nitrate spike/algal bloom recently, which has cleared up brilliantly. Have tried fungal treatment already but with no success. All other minnows are in as good a shape as ever, as are my pair of dwarf gouramis and weather loaches.

Any ideas? My only thought at the moment is plain old age...? Only guessing on lifespan though

yep, old age probably being the prime suspect here. I lost one of mine 2/3 weeks ago, went exactly the way yours is above... she did get better and fatter for 1 or 2 days, but died overnight soon after.

I know it's sad to lose a fish, but it is also rewarding to lose one due to old age. Dieing of old age is a sign that you have done everything to keep them properly. I recently lost an angelfish to old age. Hated to see her go, but she was happy and healthy all of her life.
