Fish Crazy

Right,
I've now lost three fish in three days.
2x Glowlight tetra and 1x bronze cory.
The deaths are really odd.
Three or four times a day, with at least once in the morning, the fish get a good dose of looking at to check everything is OK.
I can account for every fish, and can tell 90% of them apart, so I thought everything was fine.
However, I've now lost three fish in three days of who knows what.
There are no real symptoms. Everything looks healthy and eats well.
It just happens that between one check and another they die.
The really odd thing about them is that within this space of 5-6 hours, the fish end up looking like they've been dead for a week, or maybe two.
The eyes are buldging, the flesh is white and slimy, the fins are torn and look damaged.
THESE FISH LOOKED PERFECT A FEW HOURS BEFORE!
What on earth could be causing this?

Amm: 0
Ni: 0
Nitrate: 25
pH 7.6
hardness: pretty hard ( sorry no proper states, only dip sticks )
