Small Yellowish Worm Seen In Tank

First off I do not know my parameters
Tank is a 20g
fish: gold barbs, black ruby tiger barbs, cory cats, otto, snail
set up for about 2.5 years
Last addition to tank was a piece of driftwood about 6 months ago. All fish have been in there since initial setup.
twice monthly water changes of 25% each
Aquaclear 50 filter
Only meds used are Stresscoat for water changes
Substrate is medium sized smooth river gravel



Was scrapping some algae off and stirred up a small (around 1/4") long yellowish worm wiggling around in the water. Was not attached to anything just wiggling around and settled on the bottom and went into the gravel. Never have seen anything like that before. I assume this is a bad parasite or something along the lines. What do I need to do to rid it out of my tank. I have only seen one so far.
 
Upon further looking there are really tiny white worm creatures underneath the gravel. I can see them next to the edge of the tank. There are a lot of them. They are too small for my camera to pick up.
