Sick Betta

I am posting again because I did not know how to upload my pix. Here are some pixs of my sick fish. Can anyone tell me what to do and the right cure for him. Petco gave us Betta Revive have use it for 3days now no change yet. No sure if it need this because I do not know what is wrong with him.

Can you give us some more information on what you observe? Does he move at all? Does he breathe laboured, does he have spots? If so, what do they look like?

What do you keep him in, how much water? Does he have a heater and filter in with him? Are you changing his water? Do you have a test kit to check the levels of Ammonia, Nitrite and Nitrate? And is your tank cycled?
