So how does everyone maintain a quarantine tank? Do you just keep a spare empty tank and fill it up when you need to? Do you keep it running all the time? I have a spare 5G complete tank now, and I kind of feel like it would be nice to have a spare cycled tank already set up. But then it seems like a waste of my time to maintain an empty tank. I guess once it is cycled, it wouldn't really be any maintenance at all except to feed the bacteria. I could even throw a snail or 2 in to provide an ammonia source. What do you all do?

I've never used a QT in my tropical self. I have however used one in marine but I've never really had a issue with fish, I ought to count myself lucky.

Normally, If you need to use a QT, You remove some of your mature media from the tank you have running, Put it in a filter in your 5G tank and your cycled right away, Just keep things clean in the QT and treat whatever you need to treat. You really don't need to keep it running all the time.

However, It does come with its downside where the issue may pass through the media you have removed and go into the QT. But I find if its just one or two fish that are infection it normally isn't a issue.

as long as you are keeping a clean environment for your fish you shouldn't have issues, and when shopping for them to make sure that whatever you buy isn't sick then also no issue.
i have my 36 non-planted tank as my hospital tank.. i just add in carbon when i don't need meds in it.. it is already set up for other fish and add if there is a sick fish i add the fish in, put the meds and boom continue like before. probably not the best as the fish should be separated, but thats all i got and when the fish get sick it is just something simple to fix.

I have a large (60l or so) plastic storage box that, when it's not in use, I keep all my spare fish stuff in. I know if I had a 'proper' tank, I'd just end up keeping something in it! Then I keep the sponge from an air driven filter in my oscar's external to keep it cycled :)
