Possible Parasite In T-Bar Cichlid

Well this is the thing, this tank is a well matured 2 year old now (roughly) water quality is good, the water itself is fine (tests) but its the plants and fish, it's getting me so down and i dont know how to tackle it! ive tried breaking it down and restarting it up (keeping the mature media of course) replanted the plants, allsorts, the tank has been treated for parasites aswell,

I've got a external filter i might throw into the mix, see if that helps!

about 2 years i reckon? it wasnt to long after i got them that i moved them to the larger tank, i do have an orignal thread somewhere about them, it may give a good idea of when i got them and how old the tank really is, im just going by estimates atm!
