Please Help My Plants Are Not Doing To Good

hi everyone
I have been my planted tank for about 12 months with no problems till month or so ago, seemed to have dramatically dropped off and algae has gone wild.
its a 5 gallon tank with afew platies and t5 15w light. plants are water wisteria, elodea densa,java moss and Vallisneria tortifolia
I don't run any co2 but do use some ferts when I remember,
any help would be greatly appreciated

Lack of CO2 and ferts, that lighting is very high, your aquarium must have used up all available ferts and CO2.

which is the better way of add co2 to my tank, Liquid carbon or diy yeast set up
also what ferts would recommend

As its a small tank I'd go with some liquid carbon, 1ml a day would be double what you'd need and over dosing it slightly will eventually kill the algae, say 2ml a day.

thank you I have been use 2ml of liquid carbon a day most of the algae has gone except my java moss which has loads of algae, whats the best way of get it out of the moss
 
Continue with the carbon really, get yourself some ferts containing N&P too tho or your plants will die.
 
I have been add 1 ml of nutrafin plant gro daily but I have been looking into EI
 
I'd go with EI as plant gro doesn't contain macro nutrients which you need with carbon.
