the tank holds 350 litres no noise or kids and they are really calm only spooked when a do water changes the others are fine just this one whos doin it

So it's not alone then, all the other are fine just this one, so that would suggest a gill problem if it needs to breath rapidly

Perhaps try melafix which is a natural medication and will help to flush the gill's of any infection.

Have you done any water tests and do u do good water changes?
and can u publish your test results?

Like helterskelter mentioned, can you please give your water params? Particularly the ammonia.

Does he do this constantly or once in a while? If once in a while, is he in his territory and by the boundary of it and another piranha close by?

Parasites or water quality problems are the most common causes of individual fish breathing abnormally. Please post us your recent nitrite, nitrate and ammonia stats, the temp of the temp, behavioral and physical symptoms of the fish (particularly any unusual coloring or swelling around the mouth and gills plus body in general).
