Hello and welcome to the forum,
From reading your post it looks like your tank has not cycled yet, .25ppm nitrIte and 0ppm Nitrate leads me to this. Can i suggest you perform a large(50-75%)water change to bring down your nitrite(toxic to fish) levels.
Would i be right in thinking you are using 6in1 test strips?
Test strips have been known to be a unreliable way of testing are tank waters chemistry, myself and a lot of the members recommend a liquid based testing kit(Api freshwater master kit).
I noticed there was no test results for ammonia, this is also toxic to the fish. If your tank is not cycled there is 99.9% chance that ammonia will be present.
The fish that have died will need removing as this will increase your ammonia/nitrite levels.
Now that you have fish there is a link to an article on fish-in-cycling in my signature, bottom of this post.

Keep us all posted, Keith.

Thanks for the advice so far.

Filter back on and partial water change in hand.

Using Tetra test strips which dont test amonia - so cant report on that.

No fish in tank whilst establishing. - Cleaned every thing, tank gravel, orniments, filled tank ( picked up off floor, mopped up strengthened shelf) and re filled - treated with tap safe. Left for 2 weeks with filter running added plants - left 2 more weeks with filter and light on. Added fish.

Have had to remove some dead leaves from plant and others looking brown - dont suppose this is going to help - should I just remove plant and introduce healthyier specimen?

Read articals about cycling tank so will monitor levels more closely.

I strongly recommend a liquid test kit for accurate water testing especially when cycling. If your plant is struggling i would remove it, anything that is dead in your water will increase your ammonia level which then gets processed into nitrite both being toxic to your fish. Going back to your 6in1 test kit i myself have used this kit, not having a test for ammonia is one of its many flaws.
