Melafix Overdose?

i'm treating for tail rot on a male rosy barb. his condition appeared to improve for the first 2 days after adding melafix, but this morning i saw that he has lost some scales in the area between his dorsal fin and his head.

some of his right pectoral fin also seems to have rotted or burned away somehow. could a melafix overdose be the reason behind these 'injuries'?

i only add 5ml per 10 gallons according to instructions, but the water is always evaporating quickly (2.5 gal evaporates every 2 days). assuming the melafix doesn't evaporate (which i think it doesn't), the melafix concentration increases as water evaporates.
 
Melafix is no good on bacterial finrot should only be used for cuts and wounds.
The missing scales has the fish knocked them off on ornaments in the tank.
Any signs of flicking and rubbing.
The only way to dilute melafix is water changes.
