Lost Another Fish

my first fish I lost, well, I literally lost him. I went away for a day and a half, and one of my serpae tetras was gone. I dont' know if he died and got munched on, or if he actually got out, ( I looked but who knows).

Then two days ago I was checking out the tank and I saw something red in a crevice in the stump hide thing I have. I looked closer, and it was one of the 2 male cherry barbs, somehow he got himself wedged in this little tiny spot in the thing. So I got him loose with a net, maybe that wasn't the best way to do it, but kinda panicked and grabbed the nearest thing. He swam away looking ok, well, besides the fact that both of his side fins (don't know the correct name) and a patch of scales on his back was missing. He actually had a tiny bit more of a nub of fin on one side, so he was kind of swimming in circles, and the poor guy had to hide in the plant to keep himself from floating away. He was dead the next morning. I think it was just a freak thing, but I want to somehow fill up that gap where he got stuck just in case.
 
hey, is this crevice actually built in/part of the tank, or is it a moveable ornament/decoration?? Because if it is a fairly small space, i would probably try to fill it up with some of the substrate. Alternatively, you could try putting some plants in front of the entrance(s) to deter some fish from entering. Hope i could help!! :)

yes it's just a removeable hide that looks like a stump. thanks for the suggestions. I'm not sure that the substrate thing will work though because there are several other holes for entering the hide, unless I can find a stone or rock that will block it up. I'm not sure if it would stay on there though, I may have to just take the whole thing out and silicone a rock to cover it.
