I have some Limnophila Aquatica as a central background plant in my tank, it has now reached the surface (very fast growing plant). Do I wait for side shoots to appear or do I just take cuttings off what is already there.

What most stem plants will do is keep growing after reaching the top. They will literally just tip over as they try to extend beyond the waterline.
 
You can cut the top off when you wish. I frequently cut my stem plants to about half the depth of the tank. If you just cut the top off, new growth tends to start up there, which is no use to me.

The term "nip off" is used a lot. It implies you can simply break the top off with your fingers. This works some of the time, but also crushes the stem and causes damage which can rot. I always advocate using sharp tools to cut plants.

The piece you have cut off will grow. Let it float for a week say, so the end can heal up a bit before planting.
