Is this tail rot? What do do?

This is the 2nd time after a 30% water change that a few days later one of my forktail rainbowfish is suffering from what looks like tail rot? All other fish in my tank look fine. What could be going on? What should I do?

My first thought was clamped fins rather than TR. But I’ve never dealt with either of those symptoms before. Hopefully another member knows what it is

You say this is the second time this has happened. What was the outcome the first time? Was it the same fish or another of the same species?

It was a different fish but same species and it ended up dying several days later. It looks like its getting worse and I medicated with Kanaplex
