Holy leaves!

Why on earth would you not suspect the pleco? I've personally watched mine suck great big holes in the middle of leaves just as he described. Adding more veggie matter will definitely help. Try cucumber - that's my plec's favourite.

pleco, hands down.
we had 2 baby plecos, and within hours my plants were chomped.
we figured the one was trying to out-eat the other for terrritory issues?
we had to remove a pleco, i was loosing too many plants, too quickly.
now, the other plec is fine, and so are my recovering plants.
