Can anyone tell me what this is and how do i fix it, getting conflicting reports from my local fish store

not sure if marine fish get it but looks very much like a pretty bad case of hole in the head , usually caused by poor diet and water conditions. keep your water spot on and make sure it gets top quality diet , preferably something vitamin enriched. meds would only be warranted if a secondary infectio was present , and it doesnt look to be the case from the photo. spotless water and correct nutrition are whats needed here imo .
 
I tried enriching his diet(increased brine shrimp spirulina brine, added thera-a, and seaweed) change water weekly. I have tries this new diet. That was our first try at solving this. He ate like crazy. He ate the thera-a pellets and shrimp but would not touch seaweed. He grew big, but still kept deteriorating. My water is great, yet I don't have any live coral. Just have blemmy, hermit crabs, and 2 clowns. I am hoping add, but don't want to unless I can fix this. HELP.
 
i have to admit i am not very clued up on marine , but to me that looks exactly like hole in the head , and the answer to hole in the head is good water and nutrition. have you tried posting in the marine section ? theres plenty of experienced marine keepers there that will no doubt be much more help to you than me .
