Help! Sick Cory

My pH is at 6.8 Ammonia less than .25 and Nitrate at 5
He is breathing heavy and having difficulty staying up right. Last water change was two days ago 25% adding only 5% back at time. it's a 40 gallon aquarium with another cory, a loach, a pleco, and an angelfish. He was the newest fish but is almost a year old. I noticed he looked a little thin yesterday, so i gave them two algae wafers last night. Today he has taken a turn for the worst. should I do a round of melefix? I feel like i am reaching here without knowing what this is...any ideas?
 
It's gotten way worse his body has become curved, and he can't really swim when he does it's flailing and whirling. I am doing an emergency water change....any help or advice would be much appreciated.
 
Large water changes, you don't really want any ammonia in there.

When you say he's a little thin, does he have a sunken stomach??
