Bring the tank temperature up to 85F, make sure there is plenty of aeration. 40mg/gallon metronidazole, with a daily 50% water change & redose. After 3 days sprinkle a little metro on some frozen brine shrimp, make it look like a tiny powdered donut. Feed this only, once daily, continue for 10-14 days.

This is usually caused by internal protizoans, a common cichlid ailment. You could probably bring the temperature up a little higher, keep an eye on aeration, as warmer water holds less O2. I treated an oscar this way, tank temp near 90F, angels can handle 95F no problem.
