If you look at the picture of the underside of your plec it appears slightly emaciated (thin) and has a golden/red tinge to it. This is almost always a sign of a stomache infection or malnutrition. It may be that he either wasn't getting enough algae, or enough diet supliment from sinking wafers or fresh veg.
It is a shame he died, but it probably wasnt your fault, alot of plecs are sold already in a condition of malnourishment, if you buy another, be sure to check that it's belly looks fat and light pink, not red or discoloured.
