Dwarf Gourami In The Bottom Of Tank

i have a 20 gallon tank and about two weeks ago i noticed my gourami had sores so i treated him with melaflix and the sores went away and i also added some salt to the tank but since yesterday he's been at the bottom and he's not eating. please help

can you let us know the water test results for ammonia , nitrites , nitrates and ph please
also what other inhabitants are in the tank

I tested the water last night and it was perfect. I have 4 neon tetras 5 plattys one angel one cat fish and a guppie. the gourami is looking worse, barely moving and just sitting at the bottom.
