Dalmation Molly

My dalmation molly has developed 2 white patches on each gill. I've also noticed that it is almost completely black. Can anyone tell me what is wrong with it and how to solve the problem?
My tank stats and stock is in my SIG.

Water stats
Ammonia 0
Nitrite 0
Nitrate 20-40.
Using ei ferts for live plants. Dosing excel flourish daily with 3ml.
Tank was cycled fish in, and been cycled for 3-4 weeks. Molly has been in the tank for 10 weeks.

It doesn't have white spots all over it. No other fish have white spots. It is now being nibbled by my platys.will try and get a picture tomorrow.
