Cloudy Water In Established Tank

I saw someone ask a similar question to this but I have a few differences and was hoping someone could help me.
Yesterday I noticed my fishtank was just slightly cloudy. So I did a 25% water change. The water remained cloudy even after the water change. I was hoping it would clear up overnight, but when I came out this morning it was almost completely white. I freaked out and opened the lid. My two 4 year old big Raphael Catfish were floating at the top. I ran and grabbed a cup to scoop them out, but thankfully when I nudged them, they were still alive.
When I did the waterchange yesterday, I also tested the water. PH, Ammonia, Nitrates, and Nitrites were all normal or low. Normally my Nitrate level is a little high so that confused me.

Anyway, after seeing my Raphaels in trouble, I did a 60% water change. The water is still quite a bit cloudy. I also was devastated to discover I lost a 2 year old Bleeding Heart Tetra, my 2 two year old Kuli Loaches and my huge 2 year old Clown Loach. They still were quite bright colored so I assumed they died pretty recently. I actually kept them in seperate cups for a while hoping they were in shock and not actually dead, but after a half hour they hadn't moved. :(

My water levels are still normal, I have no idea what is wrong. I did replace my Eclipse filter this morning as well. My BioWheel is fine and I have never replaced or cleaned it. I haven't added anything odd to the tank and no poison could have gotten in as far as I can tell. Things seem to have been as normal as thy've always been.

Here are the details of my tank:

Size of tank and amount of fish: I have a 38 gallon tank and I have now two 4 inch Raphael Catfish, two small 1 1/2 inch Clown Plecos, one 4 inch regular pleco, two 2 inch Black skirted Tetras, two 1 inch Dwarf Golden Gouramis, and three 1 inch Hatchet Fish. I also had two 3 inch kuli Loaches, one fat 3 inch Clown loach and one 2 inch Bleeding heart teatra till they died this morning.

How long has your tank been set up? About 6 years

How often and how much water do you change at a time? I usually do a 25% water change once a month.

What type filter system do you have running? I have the Biowheel and the Eclipse 3 system,

Has the filter cycled yet? and do you have filter media/ material like a bio-wheel for the beneficial bacteria to live in so that the nitrogen cycle or "biological cycle" is not destroyed with water changes & routine maintenance? The tank cycled many years ago. The Biowheel has never been changed or cleaned.

Could you test your water and give me the ppm readings for nitrates, nitrites, ammonia, Gh, & Kh along with the Ph and temperature of the water?

The levels are at normal meaning:
Nitrates: 10
Nitrites: .25
Ammonia: .25 but just spiked to 3 today after the 60% water change so very high and not normal
Phosphates: .5
PH: 7
Temp: 79

From the tap, do you have hard or soft water? I'm in Colorado, I think our water is pretty normal but on the hard side.

What do you use to condition the tanks water? I normally use AmQuel Plus, but I was out of it so I used Ammo-Lock yesterday and Stresscoat today. This was the first time I've ever used AmmoLock but I've used Stresscoat a bunch in the past.

Do you add anything else to the water? Normally no. The past week I had been using a little Melafix because one of my Kuli Loaches had a bit of a growth on his head. I've used Melafix many times in the past with no problems whatsoever.

What kind of substrate are you using and is it planted with live plants? I have plain purple fishtank gravel thats been in there for 6 years. I have 2 live plants that have been in there for at least a year.

What kind of food and how often and how much do you feed? I do vary my fishes food. I just finished off using TetraMin and got some new food last week. I'm using OSI Freshwater Flake, Hikari Sinking Wafers and Boyds Vita Diet. I alternate between the OSI and Boyds everyday and don't feed that much. I use the sinking tablets for my bottom feeders 3 times a week.

I am completely devastated. I have no idea what to do. We've always had a healthy happy tank so I have no idea what could have happened. :( Any help would be appreciated, and quickly too since my fish are not in the clear yet.

I don't know too much about this, but perhaps you cuold call your local water people, they might be working on the water lines, and things may have gotten mixed in?

I don't think it's our water because the water in the tank started turning clody before I did a water change.
I'm visiting my local fish pro in a few hours. I'm hoping he can help me.

hmm, i dont think its the ammo lock cos i use that. what about air freshners in the room where they are or other chemical pollutants? only cos your loaches went first and cos they are scaleless they feel any poisoning more than the others.
something similar happened to me and i lost my loaches first and my water went cloudy, and it was all due to a glade plug in air freshner

I do use plug ins and wax tarts in warmers but I've used them for years and never had a problem.

I am so upset about my loaches, I loved them dearly. They had the best little personalities. I am very attached to my fish so this really was a heartbreak.
I thought it was due to them being scaleless as well which is why I was shocked my Bleeding Heart tetra died. She was a healthy fish who looked to be perfectly fine yesterday.
 
Added info:
I talked to my fishpro on the phone.
He thinks it's a bacterial bloom. He also thinks I may have gotten a bad bottle of Melafix? I used the last of my old bottle last week and got a new bottle on Thursday... so I used the new bottle for several days (stopped using it this past Saturday).
Has anyone heard of Melafix going bad or being off?

Anyway, he's asking me to bring in a cup of the tank water so he personally can test it for the usual level tests, and he also has a certain type of bacteria (I foget the name, sorry) that he says will help my tank even out if I use it. It may take 24-72 hours for the bacteria levels to calm down.

Any ideas on how to keep my fish safe and happy in the meantime? They seem to be gulping alot so I think I will buy more tubing and add some extra air to the tank since I have a spare pump and airstone.

Also, should I be feeding them?
 
One of my little clown Plecos just died. And my striped Raphael is gasping and not moving much.
Any ideas to hlep the remaining fish? :( This is the worst thing I have ever had to deal with. :(

See if your LFS will keep them for you for a few days
 
i used plug ins for a while. maybe its just one of them things thats happens out of the blue. but its very traumatic when it happens. i bought a new tank and moved them but other than that i dont know.
 
I honestly think it was the Ammo-Lock. I've never used it before and I may have put in a little more than I should have. I think that decreased the ammonia too much which caused the tank to go into cycle mode again.
Would that make sense?

Sadly I lost my clown pleco, my Striped Raphael, 2 Black Skirted Tetras and a Hatchet Fish last night. I am beyond devastated at this point.
I had 14 fish 2 days ago, now I'm down to 6. :(

My local fish pro had me add that "Cycle" liquid to it hoping it'll make the tank cyle faster. RIght now it's just gotten cloudier over night. My other Raphael isn't looking so hot. :(

My levels now:
Nitrites: 0
Nitrates 5-10 (I'm normally around 30-40 so this is very low for my tank)
I did two tests for the Ammonia since I have 2 different brands on hand...
Ammonia: one test says it's .25
and the other says it's 8 (the highest level on the chart)

Also, my tank is developing a bit of foam on top, should I skim it? Also, the water is smelling funky and making my whole house smell. I'm sure it's because I've had 8 fish die in the past aday, but I have taken them all out before they started decaying or getting eaten. Anything I can do?
The fishpro told me not to do anymore water changes since it'll just make the cycling process take longer.
So what are some of your ideas?
