Live plants size of tank filter no filter water changes etc

As much as this is a beautiful idea, I don't think your Grandma would be able to cope with looking after a fish tank. There is no way she could have a tank without having to do some sort of water changes, and if she's ill this might be a bit of a problem for her :(

I know that she will not be able to care for them but I will take care of the Betta and bring it home when we are done visiting her because we live 5 hours away from her I just want to make it in her honor

A 5 hour journey is a long time for a fish to be bagged up. How many times did you plan to be visiting her?

Just once

If you're willing to take the risk, then you can do it, but a 10 hour journey after being chased round the tank and bagged would be pretty stressful for him. Also, there wouldn't be a tank for him to go into on the other side, unless you're planning to take the tank with you, and then you'd have a bit of hassle with the filter media staying alive over the journey.
Honestly, I think it'd be too much hassle for you, and you'd be better off just spending time with your Grandma rather than worrying about the fish :)

We are going to buy it where she lives then take it home so it is just 5 hours and where i am going to get them they live in bowls already can u give me ideas what the tank should look like

As for tanks, you'll be setting it up at home, right?
You'll need to be doing a fishless cycle before you get him. You can read up on them here.
Size wise, you should be looking for something along the lines of 20-25L for a single betta. He'll need a heater and filter, but the decor is up to you. I personally like black gravel with live plants and bits of nice wood in them, but of course, you'll have to design the tank to your own taste!

When you transport him, you'll need to think about how you're going to keep him warm, and keeping the water stable. All this could count into keeping him calmer, and ultimately keeping him alive. You could probably get a box and fill it with something soft like pillows and blankets, then put in the bag in the middle of all the pillows and blankets, then cover the bag with another blanket to keep it dark and warm for him.

Just throwing this out there...
When I was visiting another state this summer, I found (in Walmart of all places) a gorgeous betta that I simple couldn't pass up. We kept him in a 2 gallon pitcher for about 3 days before we left to go home- a 7 hour journey. We put him in a large mason jar with netting over the top and packed him into a box with cushioning around him to possibly help absorb any rough terrain (and also any slight splashes of water out- which there was none). He did great and is now happily living all by his lonesome in my 6 gallon filtered tank.
But years ago I traveled with my betta (when I was a kid) and he didn't survive. I think it had a lot to do with his age- he was 3.
All in all, you gotta realize that a betta- a pet- is a complicated gift. While he's at the hospital, he needs to be taken care of. Who would be doing that? And afterwards, he needs a good permanent home! There's a lot more to having a betta than just sticking him into a small bowl and feeding him whenever you remember. They are awesome fish- the smartest, in my opinion- and they deserve to be treated as such.
Good luck in what you choose. Just be responsible with your actions.

I give my bettas water changes weekly and feed them every day also I do 75 % water changes when needed I will take care of it while it is at the hospital 3 days only then we take it home

So your question is not whether to do this (you seem to have already decided) but as to what the tank should look like?
Then I would keep it simple.
Get a fish bowl (at least 1 gallon!) and get those glass marble thingies (round or flattened) and put those in the bottom. Make the marbles a pretty accenting color with your betta- for example: if your betta is blue with some pretty red accents on his fins, get dark red marble thingies to put on the bottom.
You can even add a green plant (make sure it is fabric [soft], not plastic) if you want. But since this is only going to be his setup for 3 days, I'd just go simple and pretty and inexpensive.
As soon as you get him home, put him in a bigger filtered tank! He needs more space than 1 gallon to live comfortably and happily.
Hope that helped.
