Can anyone help me with what might be wrong with it?

Stats are Ammonia 0, nitrite 0, nitrate 5 or less, Ph 7.4. Lives with another oto and 3 dwarf puffers.

Could it be IPs? Do otos get IPs? The puffs were treated with their standard IP treatment 2 months ago. It vanished for a bit as otos do and I have just seen it looking like this. It lives on fresh cucumber and the algae in the tank mainly.

Last thing I want is for it to set my DPs off with anything, but I am rather concerned about the little guy.

Any suggestions would be gratefully recieved as I currently have it netted off in the tank mid clean

that's really bad. It looks like a massive internal problem, either a blockage or bacterial infection. I'm thinking more bacterial because the eyes a sticking out as well.
I would probably put it down. At least move it to an isolation tank so the others don't eat it if it dies.

Thanks. The general concensus is put it to sleep :(

Shame but I don't want it suffering or passing anything on.

RIP little oto :(
