Betta Not Swimming So Well

I've just got in from work and noticed my Betta Splenden was laid on the bottom of the tank between some plants. This is sometimes normal, but for him to stay there when i touched the glass and then put my hand in to give him a shove isn't. He doesn't normally spend a lot of time near the front of the tank, he normally lurks about at the back.

I've pulled him out and got him in a tub (couldnt find my breeder trap) so he is closer to the air. I put a micro-pellet of food in and he gobbled it, but he can't seem to right himself. He's leaning over slightly and isn't swimming around like normal.

Should i set up my hospital tank and put a low amount of water in? I have some salts too, would this help?

He lives in a 125L community tank, water stats are 0 ammonia and 0 nitrites. PH is at 7.4 and i have no nitrates test left. The tanks heavily planted though and i dose plant fertilisers every single day. Ive had him for around 10 months. Other fish seem fine so far

Edit: i should add he isn't bloated and there is no pine coning on his scales
