Betta Help Please!

For the past two weeks (since my female betta dissapeared) I've been neglecting my male betta Jack. I would've have noticed if he hadn't ignored his food this morning. He has clamped fins, trouble swimming, black spots on top of his head, and golden specks around his face. He is on a shelf over 4" so I can't typically see him from the top. He also has little white stringy looking things attached to his sides. I'm pretty sure it's all parasites. Here is a picture: IMG_0809.JPG
I have Maracide, and Coppersafe for parasites. Please tell me how bad this is! I'm deffinitely going to be upgrading his tank asap, and I have the right filter cartridge for the new tank from the tank where my female dissapeared from, can I use that to cycle the tank (still a little confused about filter media, and such.) I really can't bare to lose a fifth betta fish in six months :( I successfully treated ich once, and saved one fish (the missing one), so I know this medicine works!
