Betta Dead, Looks Like It Took A Nose Dive.

I have a betta that I have had for almost a year. He was in a tank by himself and nothing has changed since we put him in there. This morning I looked at him and he is sitting on the bottom of the tank head down on the gravel with the front half of his body laying on the gravel and his tail and the back half of his body is in the air like he took a nose dive and crashed but didn't finish landing. I know the description sounds bad but I don't know how else to explain it. Has anyone had one like this? The fish I have lost in the past have either been floating on the top or the whole body was laying on the gravel. His belly looks a little swollen but I do not know if it came after death or before. Last time I looked at him he looked fine and was floating in his corner in the leaves waiting to be fed. I didn't check on him last night. Any help is appreciated.
 
What size tank?
Is this tank filtered?
How often did you do water changes?
 
Sounds in bad shape.
What do you feed the fish?
All you can do for the fish is an epson salt bath a teaspoon to a gal, or if the fish is on its own add a teaspoon to a gal in the tank.
Shelled peas, and a bacerial med.
