Could be a number of reasons. From the gravel or rocks being sharp and his catching them on them. Or it could be a fungal infection. In which case he'll need treating. Check your water stats and check the tank for any possibly suspects. Bear in mind the other tank inhabitants and the tank size. All will play a role in the condition of any fish.
