A couple of days ago something white started appearing on my angelfishes, and as it manifested as white spots I thought it was ich at first (which I have encountered and treated several times). However, after unsuccessful treatment against ich I noticed these distinct differences between this case and ich I have seen before:

- appears ONLY around the eyes (does not appear on fins or the body even in the advanced stages of the disease), above the eye at first and below the eyes as well when in final stages
- it is SYMMETRICAL on both sides of the head (!!!) - this is the most peculiar thing

- the number of spots does NOT change
- it appears in the SAME shape and layout on ALL infected fish (~12 fish so far)

- the spots are not actually spots (at least while there is no secondary infection), but rather as something being secreted or grown from the inside (it can grow to 4-5mm in length)
- the fins are in EXCELLENT shape, no rot or excess mucus
- does not change position (e.g. it does not fall of and reappear someplace else)
- does not react to methyl-blue/ich cures

If anybody has any idea what this is I'd be grateful, it's very demoralizing to watch the fish die one by one because I don't know what to treat them for :(

Get remidies that treat, whitespot(Ich) , velvet etc etc you can get all in ones, i use this an awful lot and it helps

looks like hole in the head (hexamita) which attacks the sensory pores of fish.
How many fish altogether, size of tank and how long have you had them?
Its more likely to appear through poor water quality or it could be a parasitic worm.
Test for ammonia, nitrite, nitrate.
How often and how much are you doing water changes?

I'm with Blackangel
it does look like hexamita

Yep, I've burrowed through a bunch of sites and it seems more and more likely that it is HITH/hexamite infection, it looks quite a bit different than on discuses get it :( Here is an image of how it looked in it's early stages:

if you think they are going to die, the best way we found of putting fish out of there misery is clove oil. we had to do it with two loach, they had white spot and then got secondary velvet and fin rot, bless them.

I don't think the poor fella is going to live much longer. I think its to far gone if with treatment. Have you removed him from your main tank -_-

Do more water changes with gravel vacs and keep the filters clean. It can go with major improvements in water quality, but if its the worm which i cant tell then you will need a parasite med specifically made for it.
